Your First Choice: Extra Benefits to Help You Get and Stay Healthy!

As a First Choice^℠ Medicaid managed care member, you keep your standard Healthy Connections benefits and get certain extra benefits at no cost to you!

Extra benefits and services (value-added benefits)

Vision

• Adult vision care, plus an eye exam and glasses every two years. See additional information.

Rewards for healthy choices

• Get a $25 gift card for completing a new member orientation within 30 days of enrollment with First Choice.**
• Get gift cards and other rewards for completing qualifying healthy behaviors.*

Maternity support program

• First Choice’s Bright Start^® program for pregnant members.
• Maternity Peer Support program is available for all expecting and new mothers (prenatal through 12 months postpartum). The program offers respectful peer circles and community-based supports. The program uses lived experience; trusted local networks; and trained peers, including doulas/community health workers and lactation peers.

Doula and lactation services are available. Prenatal and postpartum doula services provided at no cost to qualifying members.

• Local regional baby showers with choice of car seat, mobile portable crib, or stroller. Qualifying members can also get diaper bags and more. (While supplies last.)
• Electric breast pump for new and expectant moms. See additional information.
• Transportation vouchers for qualifying pregnant members. Uber must be available in the pregnant member’s local area.*
• Meals for qualifying new moms after they give birth to their child.
• Newborn circumcision for babies up to 6 months old.
• Access to online parenting classes. Online workshops to help parents solve issues, like sleeping through the night, potty training, and more.
• Keys to Your Care^® texting with tips for expectant moms. (Messaging and data fees may apply.)

Additional programs

• First Choice Take Flight — Enhanced foster care program for youth who are aging out of foster care.
• Youth Sports Program/Swim Lessons YMCA Voucher — In collaboration with select YMCA locations, qualified members can get a First Choice voucher for a choice of swim lessons or Youth Sports Program.*
• Feminine hygiene kits available at no cost to qualifying members ages 10 to 18.
• Personal safety emergency kit — These kits include road danger tools, first aid goods, and a flashlight. Contact us to get a kit mailed to you.**
• Sports checkups for children ages 5 – 18 — Members who get their sports checkups at the same time as their well visit will receive a $25 gift card. Limit one sports checkup per year per member.*
• First Choice Community Center —  The Center is a hub for the community, a gathering place where you are always welcome. 
• Fresh Fruit and Vegetable Boxes are home-delivered at no cost for qualifying members.*

Wider Circle works with First Choice to give community members the support they need to live happier and healthier lives.

Educational assistance programs

• College scholarships and GED vouchers for selected qualifying members.
• Back-to-school events with school supplies and haircuts for grades K – 12 (while supplies last).
